Enrollment for grades 9-12 (High School)

Registration for the Uintah School District online school program is  open  for grades 9-12 for the 2012-2013 school year. 

All prospective students must go to Ashley Valley Education Center to fill out a registration form.

Information on Senate Bill 69:  Senate Bill 69 basically gives students who are enrolled with their regular high school grades 9-12 the option to take two of their courses from an approved online school.  We are approved to service those students.  For more information on Utah Senate Bill 69, click here.

We must have a  birth certificate, immunizations form, signed Handbook, Home Language Survey and Proof of residence (phone bill, gas bill etc.)  for each child before we can complete enrollment.  The following are links for some of these forms.
